What to do:
1. Follow the link below to ooShirts.com

2. Create a design for the front (and back if you choose). Three logo types have been provided. You only need to use one.
3. Save your design. (The website will ask to send the design to your e-mail address.)
4. Forward your design to us at theformrocks@gmail.com
Contest rules:
- All entries must be received via e-mail by March 31, 2012.
- One winning design will be chosen, by the band, on April 3, 2012. The winner will be notified by e-mail.
- Maximum of 3 entries (or designs) per person.
- At least one of the provided "The Form" logos must be used in the design.
- The web address TheFormRocks.com or www.theformrocks.com must also be used in the design.
- No trademarked or copyrighted images or titles are allowed. We don't want to get sued.
